The job hazard analysis

PPE required: Hard hat, Safety glasses, Gloves, Safety boots

  1. 1. Check the tag and inspect

    What could go wrong

Using a scaffold that isn't ready

    How we control it

Read the tag. Walk the access route and look for missing planks, guardrails, or ties.

  2. 2. Climb on

    What could go wrong

Falls from the ladder or access

    How we control it

Use the built-in stairs or ladder, not the frame. Keep three points of contact and don't carry loads that stop you holding on.

  3. 3. Do the work

    What could go wrong

Overloading; falling tools and materials; leaning out

    How we control it

Stay within the load rating. Keep the platform tidy. Keep tools tied or in a tray. Don't lean out over the rail; move the scaffold instead.

  4. 4. Leave the scaffold

    What could go wrong

Materials left up to fall in wind; unauthorized use

    How we control it

Clear the platform. Report anything damaged. Don't change the scaffold yourself.

  • If anything looks wrong, stop and tell your supervisor. Don't fix the scaffold yourself.

When to use it

Use it for crews who work from a scaffold, and review it when the work or the scaffold changes.

Review it with the crew before the work starts, change anything that doesn't match how you'll do the job, and update it when the method, the equipment, or the site changes.

Use this JHA in Reldro

  • It arrives ready to edit, and your crew uses it on their phone, even without signal.
  • Failed items become corrective actions with an owner and a due date.
  • Every completed one is kept with the site, the date and who did it.
