The job hazard analysis
PPE required: Hard hat, Safety glasses, Gloves, Safety boots, High-visibility vest
1. Check the forms and the access
What could go wrong
Form failure; slips and falls
How we control it
Have the formwork checked before the pour. Keep walkways clear and fit guardrails at edges.
2. Set up the truck or pump
What could go wrong
Truck or pump tipping; hoses whipping; overhead lines
How we control it
Place on firm ground with outriggers on mats. Check overhead clearance. Keep people out of the swing and the line of the hose.
3. Place the concrete
What could go wrong
Skin burns from wet concrete; strains
How we control it
Wear waterproof gloves and boots. Wash off wet concrete right away. Share the work to avoid strains.
4. Watch the forms
What could go wrong
Blowouts and leaks
How we control it
Keep someone watching the forms during the pour. Stop if anything moves.
5. Clean up
What could go wrong
Residue splashes; wash-out hazards
How we control it
Wash out equipment in the designated area. Wear eye protection.
- Wet concrete is alkaline and can burn skin. Rinse with clean water and get help for irritation.
When to use it
Use it before every pour, and review it when the method or the placement area changes.
Review it with the crew before the work starts, change anything that doesn't match how you'll do the job, and update it when the method, the equipment, or the site changes.
