The job hazard analysis

PPE required: Hard hat, Safety glasses, Gloves, Safety boots, High-visibility vest

  1. 1. Check the truck

    What could go wrong

    Brake, light, or tipper failures

    How we control it

Complete a pre-trip check. Report defects before you start.

  2. 2. Load

    What could go wrong

Overloading; spillage; people near the load

    How we control it

Load evenly and within the rating. Keep ground crew out of the truck's blind spots and the loader's swing.

  3. 3. Haul

    What could go wrong

Rollovers; other traffic; poor road surface

    How we control it

Follow the haul route and speed limits. Keep the haul road graded and watered if dusty. Use a seat belt.

  4. 4. Tip

    What could go wrong

Tipping over; overhead lines; people nearby

    How we control it

Tip on level, firm ground. Check overhead before you raise the body. Keep everyone clear.

  • Lower the body completely before you drive away.

When to use it

Use it for any haul route on or off site, and review it when the route or the weather changes.

Review it with the crew before the work starts, change anything that doesn't match how you'll do the job, and update it when the method, the equipment, or the site changes.

Use this JHA in Reldro

  • It arrives ready to edit, and your crew uses it on their phone, even without signal.
  • Failed items become corrective actions with an owner and a due date.
  • Every completed one is kept with the site, the date and who did it.
